In some teeth, decay or fracture extends below the gumline and close to the supporting bone. If the tooth can be restored, clinical crown lengthening reshapes the gum and bone to create crown exposure for restoration, so enough healthy tooth structure is visible for a stable tooth restoration.

Crown lengthening may also be used for aesthetic reasons when front teeth appear short and the smile shows excessive gum display. The treatment plan depends on the position of the gumline, the bone level, the desired smile shape, and the final restorative or aesthetic goal.

Crown lengthening

Case of crown lengthening in the anterior maxilla. The teeth were used in a greater prosthetic plan (PFM bridge from canine to canine). The teeth were given the necessary biological width and prosthetic height.

Crown lengthening in the aesthetic zone

Crown lengthening in the aesthetic zone was performed using a transparent surgical splint. The gingival margin was reshaped with guidance from the splint, which was fabricated according to diagnostic waxing. The left upper lateral incisor (#22) had to be removed in a second stage.
